BlueCare, part of the UnitingCare family, seeks a Clinical Care Partner to oversee and coordinate complex clinical and care needs in community settings across Toowoomba North, Oakey and surrounding areas.
You will travel between client sites, develop and review care plans, monitor outcomes, and support clients in navigating care pathways with a focus on person-centred, high-quality care.
Registration with AHPRA is essential, with a current driver's licence and a vehicle for work.
