Assistly is seeking an experienced Level 2 Support Coordinator to join our team in Braybrook, VIC ****.This is a contract position supporting NDIS participants to understand and implement their plans, connect with appropriate services, coordinate multiple providers and work towards their individual goals.The successful candidate will already have practical NDIS Support Coordination experience and be confident managing a participant caseload independently.Previous experience working as an NDIS Support Coordinator is mandatory.Key responsibilitiesIndependently manage a Level 2 Support Coordination caseloadSupport participants to understand and implement their NDIS plansCoordinate allied health professionals, service providers, family members, nominees and mainstream servicesSource appropriate providers based on participant needs, preferences and goalsMonitor participant budgets, funding utilisation and service agreementsIdentify and resolve service gaps, provider breakdowns and funding concernsMaintain regular communication with participants and relevant stakeholdersPrepare supporting evidence and documentation for plan reassessments and reviewsCommunicate effectively with the NDIA, plan managers and other stakeholdersMaintain accurate, timely and compliant case notes and participant recordsMonitor participant progress towards their NDIS goalsIdentify and escal... safeguarding, funding or service delivery risksAttend case conferences and stakeholder meetings where requiredAdvocate for participants within the scope of Support CoordinationMaintain professional boundaries, confidentiality and participant choice and controlContribute to continuous improvement and high-quality Support Coordination practices within AssistlyMust be able to demonstrateIndependently manage a standard Level 2 Support Coordination caseload end-to-endCoordinate allied health, providers,
