We are seeking a compassionate Enrolled Nurse (Care Partner) to deliver high-quality, person-centred care in clients' homes. This role combines hands-on clinical care with monitoring client wellbeing to ensure safe, consistent and responsive care.

About the Role

The Care Partner delivers high-quality, person-centred care in clients' homes, providing clinical support, monitoring wellbeing and maintaining accurate documentation to ensure safe, responsive and consistent care.

Key Responsibilities

- Deliver personal care and in-home support in line with care plans, promoting independence and wellbeing




- Monitor client wellbeing and escalate clinical concerns, risks or deterioration via established escalation pathways
- Contribute clinical observations to support care plan development and review
- Support implementation of care plans, reinforcing safe, consistent and person-centred practices
- Maintain accurate, timely clinical documentation to support compliance and continuity of care
- Identify and report incidents, hazards and safeguarding concerns
- Build trusted, professional relationships with clients, families and care teams

About You

You are a compassionate and reliable care professional who takes pride in supporting others to live with dignity and independence. You bring strong communication skills, sound judgement and a genuine commitment to safe, person-centred care.

To Be Successful You Will Have

- Relevant qualification or experience in in-home or personal care
- Current National Police Clearance (or willingness to obtain)
- Working With Children Check
- NDIS Worker Screening Clearance (or willingness to obtain)
- A valid driver's licence and reliable vehicle
- Evidence of current mandatory immunisations
- Full right to work in Australia
