This is a community-based role where you will carry your own caseload and work with people over months and years within their own environment, making clinical decisions with multidisciplinary colleagues nearby.

About PDC Health Hub

PDC Health Hub is a community-based, multidisciplinary diabetes and allied health clinic that has been supporting people living with diabetes across Perth since 2015. Eight disciplines work together within one clinic, collaborating on shared caseloads day to day.

We are based in East Victoria Park with satellite clinics across the Perth metropolitan area including Bedford, Canning Vale, Joondalup, Kalamunda, Midland, Murdoch and Palmyra, Kojonup plus telehealth to regional and remote WA. Our model is built on self-management education, health literacy and real collaboration, supported by a diabetes technology program covering CGM, insulin pumps, smart pens and data platforms.

The role

You will join our existing CDE team, delivering evidence-based diabetes education across the full scope of practice in a community setting. Depending on your experience, your week may include:

- Comprehensive diabetes assessments covering clinical, psychosocial and behavioural needs, for people living with Type 1, Type 2, gestational diabetes, LADA and MODY

- Individualised self-management education including goal setting, behaviour change support and medication education

- Managing your own longitudinal caseload, seeing the same people repeatedly rather than in a single admission

- Insulin initiation and titration in the community, working alongside the person's GP and around their food, work, shift patterns and family commitments

- CGM education and support across Libre, Dexcom and other systems, including data interpretation and troubleshooting

- Insulin pump starts and follow-up for Omnipod, Tandem, Medtronic, MyLife Diabetes and emerging technologies, after completing our internal training program

- Sick day education, psychosocial screening and support

- Working within primary care funding pathways including GP Management Plans, Team Care Arrangements, chronic disease items, NDIS and DVA





- Facilitating group education sessions and contributing to resource development

- Participating in multidisciplinary case conferences and clinical governance activities

- Telehealth consultations for people across regional and remote WA

- Contributing to program development, quality improvement and health professional education

What we are looking for

Essential:

- Current Credentialled Diabetes Educator (CDE) status with the Australian Diabetes Educators Association (ADEA)

- Current AHPRA registration in your base discipline (Registered Nurse, Pharmacist, or other CDE-eligible profession)

- Demonstrated experience delivering diabetes education in a community, primary care, private practice. Applicants from other settings are welcome to apply and are asked to outline how their experience translates to community practice.

- Experience managing your own caseload with a high degree of clinical autonomy but also supported by your team lead.

- Capability to work with people in the context of their daily lives, where cost, food security, work patterns, transport, family and motivation shape what is achievable

- Medicare provider number, or eligibility to obtain one

- Current professional indemnity insurance

- Clinical experience across more than one diabetes type

- Sound knowledge of current diabetes management guidelines and pharmacotherapy

- Solid communication skills with a person-centred, health-literate approach and non judgemental

- Ability to work collaboratively within a multidisciplinary team

- National Police Clearance and Working with Children Check, or NDIS Worker Screening (or willingness to obtain prior to commencement)

Desirable:

- Experience working within Medicare-funded chronic disease pathways, NDIS or DVA





- Established relationships with GPs and primary care teams, or experience communicating clinical recommendations back to referrers

- Experience with CGM, insulin pumps and hybrid closed-loop systems in an outpatient or community context

- Experience delivering care through home visits, residential aged care or telehealth

- Complex case management experience

- Group facilitation skills

- Involvement in research, clinical audit or quality improvement

- Interest in clinical leadership or advanced practice pathways

- Nurse practitioner or workings towards
