About the role
We're looking for a friendly and passionate CDM Nurse to join our team on a full-time basis at Myhealth Auburn Clinic. This is a fantastic opportunity for a nurse who enjoys building long-term patient relationships and improving health outcomes through preventative care and chronic disease management.

Our position offers a Monday to Friday schedule during office hours, allowing you to achieve a fulfilling work-life balance.

A day in the life:

- Work alongside GPs to support patients with chronic and complex health conditions
- Develop and coordinate individualised care plans in collaboration with patients, GPs and other healthcare professionals
- Conduct Health Assessments and reviews
- Support patients with education to encourage engagement in managing their health conditions
- Mentor and support the nursing team in CDM processes
- Work with the clinic team to monitor CDM activity, identify trends and contribute to improved patient health outcomes
- Maintain accurate clinical documentation and patient records
- Other related duties as required

What we're looking for:

- Current AHPRA Registration as a Registered Nurse
- Demonstrated experience in Chronic Disease Management within a General Practice setting
- Strong clinical knowledge of chronic disease management and preventative healthcare




- Excellent communication and strong customer service skills
- Ability to work autonomously when required
- Strong multitasking abilities and effective prioritisation of workflow
- Best Practice software experience (desirable)
