We are currently seeking a compassionate and customer-focused Client Liaison Officer to join our Sunshine Coast Aged Care at Home team in a permanent part-time capacity.As the primary point of contact for many of our clients and their families, you will play a vital role in ensuring older Australians receive the services, support and care they need to continue living well at home.About the RoleReporting to the Care Partnering Lead, you will be responsible for coordinating client care and providing exceptional service throughout the client journey. You will support new and existing clients through intake, assessment, care planning, service coordination and ongoing reviews, helping to deliver a seamless and positive care experience.Key ResponsibilitiesConduct client intake conversations, telephone assessments and service enquiriesAssist with care planning, reviews and ongoing evaluation of client needsCoordinate and schedule services to support continuity of careAct as the primary contact for client, family and representative enquiriesBuild and maintain strong relationships with clients, families and internal stakeholdersMonitor service delivery and identify changing client needs or risksMaintain accurate records, documentation and system updatesSupport onboarding processes for new clientsContribute to quality improvement initiatives and service excellenceEnsure compliance with organisational policies, aged care standards and program requirementsAbout YouYou are a caring and organised professional who enjoys building relationships and making a positive difference in people's lives.
