About the role

Support Workers providing person-centred active support to clients with disability, promoting their engagement in daily activities, community participation, and independence. The role involves supporting clients across a range of activities including encouraging positive choices in the home in particular and in the community at times this could mean joining in with personal exercise routines including gym workouts, aquatic activities, and other activities, while maintaining appropriate physical and emotional support at all times.

Key responsibilities

- Follow daily tasks, activities and schedule to ensure a protected and comfortable living environment, maintaining appropriate physical and emotional support to clients at all times, including joining them in gym workouts, the pool for aquatic activities, bike riding, etc.

- Provide support and services to clients with the Person Centred Active Support Model which promotes individuals living with a disability being engaged in their own lives

- Participate in programmes and activities that encompass the individual needs of clients to ensure participation and development of individuals that provide opportunities to extend social and personal networks

- Assist clients with their personal and health care and mobility needs by pushing wheelchairs, undertaking wheelchair transfers and use of hydraulic lifters and a range of other manual tasks

- Assist with showering, continence,



oral hygiene and meals

- Administer medication and undertake other health care interventions

- Assist with household/venue duties including cleaning, washing, ironing, shopping and meal preparation

- Advise Direct Line Report of the changing needs of clients and identify current and innovative models of service to ensure a culture of excellence in the development and delivery of services is continually achieved

- Ensure client notes are accurate and are kept up to date

- Report any accidents and incidents to the Team Leader in accordance with set policies and procedures

About you

- Queensland Working with Children Blue Card

- NDIS Worker Screening Card

- National Police Check

- Current First Aid & CPR Certificates

- Current Drivers Licence

- Understanding of Disability and Mental Health Support Needs

- Experience in providing Supported Independent Living (SIL) supports

- Manual Handling and Mobility Support

- Medication Assistance

- Person-Centred Approach
