Support at Home Care Partner – Full-Time

Ourcare Services Ltd is seeking a compassionate, organised and client-focused Support at Home Care Partner to join our team on a full-time basis, supporting clients throughout the Maitland and surrounding areas.
Ourcare Services has a proud history of supporting older people and people living with disability to remain independent, connected and safe in their own homes and community.
About the Role
As a Support at Home Care Partner, you will work closely with clients, families, support workers and service providers to ensure clients receive high-quality, person-centred care that reflects their individual needs, goals and preferences.
Key Responsibilities
- Coordinate and monitor individual Support at Home services and supports.
- Develop, review and maintain individualised care and support plans.
- Build positive and respectful relationships with clients and their families.
- Conduct regular client reviews and respond to changing needs.
- Coordinate services including personal care, domestic assistance, social support, transport, respite and allied-health services.
- Monitor client wellbeing, safety, risks and changes in circumstances.
- Liaise with support workers, nurses, allied-health professionals and external service providers.
- Assist clients to understand and make informed choices about their supports.
- Monitor service delivery and client budgets to ensure supports are appropriate and sustainable.
- Maintain accurate case notes, records and documentation in accordance with organisational and legislative requirements.
- Respond appropriately to incidents, complaints, safeguarding concerns and changes in client circumstances.
- Advocate for clients and promote their independence, dignity,
choice and participation in the community.
- Contribute to quality improvement and compliance with aged-care requirements.
About You
We are looking for someone who:
- Has experience in aged care, community services, case management, care coordination or a similar role.
- Has a strong understanding of person-centred care and supporting people to remain independent at home.
- Has excellent communication, organisation and time-management skills.
- Is confident working with clients, families and a range of health and community professionals.
- Can manage competing priorities while maintaining a high level of attention to detail.
- Demonstrates empathy, professionalism, reliability and a genuine commitment to supporting people.
- Understands the importance of privacy, dignity, choice and client rights.
- Is confident using computer systems and maintaining accurate client records.
- Has a current Australian driver's licence and reliable vehicle, where required.
- Meets all relevant aged-care screening, police check and employment requirements.
