Reliant Care Pty Ltd is seeking experienced, reliable and participant-focused Disability Support Workers to join our Supported Independent Living (SIL) teams.

We have several vacancies available, including a House Leader position at one of our South SIL homes.

Allocated Roster Requirement

These positions are being recruited to fill specific roster vacancies. Successful applicants must be able to commit to the allocated roster on an ongoing basis.

Required Support Worker roster:

Monday: 06:00 - 15:00 (Rockingham)

Tuesday: 06:00 - 15:00 (Rockingham)

Wednesday: 06:00 - 15:00 (Rockingham)

Thursday: 15:00 - 06:00 (inactive sleep shift) (Wellard)

The roster may include daytime, evening, weekend, public holiday, active-night and inactive overnight shifts and may be subject to change (although we endeavour to maintain consistency in Participants rosters).

Applicants should carefully review the roster before applying. While reasonable changes may occasionally be considered, these roles are not suitable for applicants seeking to select or regularly change their shifts.

Support Worker Responsibilities

- Provide person-centred support in accordance with participants’ support plans, goals and preferences.

- Assist with personal care, medication, meal preparation and household tasks.

- Support participants to access appointments, activities and the community.

- Implement behaviour support and risk-management strategies.

- Maintain professional boundaries and promote participant choice, dignity and independence.

- Complete shift notes, medication records, incident reports and other required documentation.





- Communicate concerns promptly to the House Leader, Coordinator or On-Call Manager.

- Follow Reliant Care policies, NDIS Practice Standards and workplace health and safety requirements.

- Work cooperatively with colleagues, families and other members of the participant’s support team.

House Leader Opportunity

One of the available positions includes additional responsibilities as the House Leader for our Rockingham SIL home. The House Leader will work rostered support shifts while assisting with the day-to-day coordination of the home.

Required House Leader roster:

Monday: 06:00 - 15:00 (Rockingham)

Thursday: 06:00 - 15:00 (Rockingham)

Friday: 06:00 - 15:00 (Rockingham)

Sunday: 06:00 - 15:00 (Rockingham)

Additional responsibilities include:

- Supporting consistent service delivery across the staff team.

- Acting as the primary link between the home and the Service Delivery Coordinator.

- Monitoring household routines, appointments, documentation and communication.

- Supporting new workers during orientation and shadow shifts.

- Identifying and escalating participant, staffing, medication, maintenance or safety concerns.

- Promoting teamwork, accountability and professional conduct.

- Assisting with house meetings, audits and the implementation of participant support plans.

- Ensuring participant information and household records are accurate and current.





Applicants interested in the House Leader position should identify this in their application and outline their relevant leadership and SIL experience.

Essential Requirements

- Relevant disability support or SIL experience.

- Current NDIS Worker Screening Check.

- Current National Police Clearance.

- Working with Children Check.

- Current First Aid and CPR certification.

- Current unrestricted driver’s licence and access to a reliable, insured vehicle.

- Medication competency or willingness to complete required training.

- Ability to complete clear and accurate workplace documentation.

- Ability to work independently and as part of a team.

- Reliable availability for the advertised allocated roster.

Desirable

- Experience supporting people with complex needs or behaviours of concern.

- Experience implementing Behaviour Support Plans.

- Experience working in a SIL environment.

- Previous senior support worker, team leader or House Leader experience.
