About the role

The purpose of the Care Partner role is to support our Support At Home (SAH) clients with the provision of consumer directed, goal orientated, cost effective and evidence-based care management services. This objective will be achieved by working collaboratively with other HIB care partners, care staff, management, the client and multi-disciplinary teams and stakeholders to achieve positive outcomes that optimise health and wellbeing in accordance with clients' needs, goals and preferences, and to assist them to remain living safely and independently at home.

Key responsibilities

- Support our SAH clients with exceptional case management services

- Work collaboratively with other care partners, care staff, management, clients and multi-disciplinary teams

- Work with and for the client in a proactive, collaborate supportive manner

- Achieve positive outcomes that optimise health and wellbeing in accordance with clients' needs, goals and preferences

- Support clients with a high level of case management and interpersonal skills

- Management of the client’s government funded budgets to ensure they continue to receive their assessed services as required to remain independently at home

- Supervise and support staff.

- Commitment to continuous quality improvement





- Always maintain accountability to the client and organisation.

About you

- Have a passion for working in Aged Care

- Exceptional Case Manager with previous demonstrated experience working with the new government funded Support at Home Program

- Experience with the new Aged Care Reforms

- Experience in staff supervision and support

- Proficient in the use of Microsoft Office 365

- Active team player who thrives in a busy, yet fun work environment

- Someone who loves a challenge and enjoys contributing to the growth of the organisation

- Committed, reliable and versatile

Minimum compliance requirements

- Minimum qualification of Certificate III in Individual Support

- Preferred qualification in Case Management

- National Criminal History Check or Yellow Card

- Current First Aid and CPR Certificate

- Registered, comprehensively insured vehicle and current licence

- Completed online aged care modules as per new aged care reforms prior to commencement
