You’ll be responsible for:
Co-ordinating appropriate care and services for clients in consultation with external agencies




Maintaining and updating care management systems relating to our clients and their representatives, stakeholders, service providers, and government agencies
Scheduling initial intake appointments
Sharing the scheduling of workers for clients
Liaising with clients and/or their representatives on aspects of their Home Care Package
Consulting with clients, family members, external agencies and other supports to ensure that care is ongoing and that all client needs are identified and met.
Timely follow up of incident reports and ongoing welfare checks
