The Registered Nurse provides secure, person-centred and culturally responsive nursing care to older people receiving Support at Home services, supporting them to remain safely and independently in their own homes.
Key Responsibilities
Conduct comprehensive nursing assessments and identify clinical and environmental risks.
Develop, implement and regularly review individualised care plans.
Monitor consumers' health and respond appropriately to changes or deterioration.
Provide clinical advice, health education and nursing care within scope of practice.
Coordinate care with GPs, allied health professionals, support workers, families and other providers.
Support medication, wound, continence, nutrition, chronic disease and palliative care needs as appropriate.
Maintain accurate clinical records and documentation.
Report and manage incidents, risks,
complaints and safeguarding concerns.
Provide clinical guidance to support workers where required.
Participate in clinical audits, quality improvement and compliance activities.
Promote dignity, choice, independence, privacy and culturally appropriate care.
Essential Requirements
Current AHPRA Registered Nurse registration.
Relevant aged care, community or clinical nursing experience.
Robust clinical assessment, communication and documentation skills.
Ability to work independently in a community/home-care workplace.
Current police/background screening and required vaccinations/checks.
Current First Aid/CPR and driver's licence where required.
