MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. We are currently seeking compassionate and dedicated Case Managerto join our team based in the Melbourne, Australia.

About the role

Exciting opportunity for a person who is passionate about aged care, and supporting the ageing and elderly. The successful candidate will have a solid client service focus with demonstrated ability to maintain strong ethics and care.

To be consider for the role you are self motivated and are passionate about making a positive difference for the ageing and elderly and you understand that the role requires a combination of both reactive and proactive measures. You thrive in a rapid pace workplace and are a great problem solver.

Some of the responsibilities for this position include:
Demonstrated ability to build robust partnership relationships with client
Provide comprehensive case management for each client
Build strong Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client’s budget
Re-assess client care plans, goals, OHS assessment, and emergency plans for clients
Maintain strong client notes




Strong problem solving skills
Nimble in their ability to support clients

About You
Ability to speak Macedonian language is required
Proficiency in computer skills
Excellent communication and interpersonal skills
Ability to work independently and as a part of a multidisciplinary team
