South Gippsland Hospital is seeking a Welfare Worker, Class 2A (Transition Care Program Care Partner) on a casual basis. The role ensures continuity of care coordination to assist frail older people and younger clients with early entry to the Aged Care system, enabling them to remain at home and in their communities.
The successful applicant will have relevant tertiary qualifications and a qualified, empathetic attitude, plus high level IT, decision-making and communication skills.
