IPC Health is seeking a Living Well Wellbeing Coordinator to improve health outcomes for people with chronic conditions. You will work in a multidisciplinary team providing direct client care and championing self-management and behaviour change across the organisation.
You will perform holistic assessments, use motivational interviewing, and help clients set and achieve health goals, coordinating care and referrals while supporting access to internal and community services.
