About the Role
We are seeking a compassionate and motivated
Support Worker
to support clients in the community. In this role, you will provide psychosocial support, assist with NDIS applications, and help clients build independence, confidence, and sustainable lifestyles.
Key Responsibilities
Support clients to develop daily living and functional skills
Provide emotional support, guidance, and problem-solving strategies
Maintain accurate case notes and monthly reports
Monitor client progress and follow up on referrals
Work collaboratively with families, service providers, and stakeholders
Skills & Experience
Certificate IV in Mental Health, Community Services or related field (or relevant experience)
Robust understanding of mental health and recovery-focused practice
Experience working with diverse and vulnerable populations
Excellent communication and interpersonal skills
Ability to advocate for clients and support access to services
Experience or willingness to facilitate group programs
Mandatory Requirements
Current NDIS Worker Screening Check
National Police Clearance




Current Driver's Licence
Working With Children Check (WA)
Desirable
Experience working in an Aboriginal Community Controlled Organisation
