community connection and referral services.About The RoleAs an Initial Assessment and Planning Worker, you'll be a vital first point of support for people experiencing homelessness or facing the risk of losing their home, helping them navigate crisis and find a path toward safety and stability.
Through compassionate, person-centred support and rapid intervention, you'll connect individuals and families with safe, affordable accommodation and the services they need to build a brighter future.
Every day, you'll have the opportunity to make a meaningful difference, empowering people during some of the most challenging moments of their lives.Reporting to the Team Leader, this is a permanent full-time role, located in Nunawading, VIC.ConditionsCompensation is in accordance with Social, Community, Home Care and Disability Services Award (SCHADS) - Level 5.How You Will Make An ImpactProvide immediate support and assessment for people experiencing or at risk of homelessness, helping them access safe and suitable housing optionsDeliver personalised short-term support, including housing applications, goal planning, advocacy, and referrals to relevant servicesBuild trusting relationships with clients, empowering them to make informed decisions and work towards greater stability and independenceConnect clients with a range of community, housing, health,
