About The Role
The role is to manage a case load of clients under CHSP/HCP approved funding.
Maintaining budgets, on boarding growth of new clients and managing care requirements to maintain clients independency.

What You Bring
Evidence of leading a fast paced teamEvidence of leadership in Case Management or senior case manager roleRelevant tertiary qualification in the community sector or Cert4 in leadership will be desirable Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needsFull Drivers Licence and own reliable registered and insured Vehicle Smart PhoneKnowledge of My Aged Care portal/assessment process Willing to work in a team environment A desire to join a team committed to the care of others in a mission based organisationNDIS clearance (or willingness to obtain one)Police clearance (within the last 6 months Or willingness to obtain one)
