Position Classification: Registered Nurse

Remuneration: $ - $ per week

Hours Per Week: 38

Requisition ID: REQ

Application close: Sunday 23rd August 2026 at 11.59pm

Central Coast Local Health District (CCLHD) has an opening for permanent full-time Registered Nurse to join the Hospital in the Home (HITH) team at Gosford, Wyong Hospitals, and the Long Jetty Health Care facility. In this vital role, you will serve as primary health care worker in the community, providing acute clinical care to patients in their homes or at HITH clinics. You will collaborate with multidisciplinary teams to monitor health statuses, respond to episodic health needs, and promote health maintenance, rehabilitation, and disease prevention.

Whilst this position is open to all individuals, it is targeted to Aboriginal or Torres Strait Islander people under section 21 of the Anti-Discrimination Act 1977. Aboriginal or Torres Strait Islander people are encouraged to apply and consideration will be given to suitable Aboriginal or Torres Strait Islander applicants to promote equal or improved access. Aboriginal or Torres Strait Islander applicants must demonstrate Aboriginal or Torres Strait Islander status in addition to addressing the selection criterion.

About The Role

As a Registered Nurse in the HITH program, your primary responsibilities will include delivering acute clinical care to patients in their place of residence or at a HITH clinic, working in close collaboration with the multidisciplinary HITH team. Close ties with both the community sector and the acute hospitals within Central Coast Local Health District will be required. As the settings in which care is delivered, HITH nurses require skilful application and adaptation of evidence-based practice on which nursing is based.

You will play a key role in driving innovation and leading the development and delivery of patient education programs, ensuring that patients and their families are equipped with the knowledge, skills, and resources needed to make informed healthcare decisions. This includes providing education on their conditions, medications, and self-care strategies, enabling them to actively participate in managing their health and improving their overall wellbeing.

Flexibility and adaptability are key,

as you will need to assess and respond to a variety of clinical situations in diverse home settings. You will actively promote health, recognise early signs of disease, and provide timely interventions to help patients maintain their independence and well-being. Additionally, you will record data for service evaluation and contribute to continuous quality improvement efforts within the team.

About You

- Recent nursing experience in Acute Care Nursing or a demonstrated interest in Acute Care nursing.

- Demonstrated ability to liaise with other health professionals to facilitate case management and care planning.

Applicants must have current work rights in Australia or be a permanent resident or citizen. Unfortunately, we cannot consider applicants who do not meet this requirement.
