In this role, you will deliver comprehensive, client-centered nursing care to individuals in their homes and community clinic settings. You will play a vital role in promoting independence, managing complex chronic conditions, facilitating wound care, and delivering palliative care across your regional community.
Key Responsibilities
- Conduct holistic clinical assessments and formulate care plans tailored to individual client needs.
- Deliver high-quality nursing care in client homes and clinic settings, including wound management, medication administration, chronic disease management, and palliative care.
- Collaborate closely with GPs, allied health professionals, acute health services, and community care teams.
- Maintain accurate, timely, and compliant electronic medical documentation.
- Educate and support clients, families, and carers to manage health conditions independently and safely at home.
Essential Requirements
- AHPRA Registration: Current registration as a Registered Nurse (RN) or Enrolled Nurse (EN) with the Nursing and Midwifery Board of Australia.
- Community Nursing Experience: Essential. Proven experience delivering autonomous nursing care within community, home care,
or district nursing environments.
- Driver’s Licence: Current Victorian Driver’s Licence (access to a pool vehicle or vehicle allowance provided).
- Certifications & Checks:
- Current National Police Check (or willingness to obtain).
- Working with Children Check (WWCC) & NDIS Worker Screening Check (if applicable).
- Current First Aid and CPR certifications.
- Strong clinical reasoning, time-management, and problem‑solving skills with the ability to work independently in the field.
