About the Role
Due to continued growth within the organisation, our Head Office, based in Balcatta, is seeking a Full-Time NDIS In Home and Social Services Coordinator to oversee and coordinate in-home, social and community services.
As an In Home and Social Services Coordinator, you will work closely with participants, families, guardians, Support Workers, allied health professionals, and key stakeholders to ensure participants receive safe, person-centred, and outcome-focused in-home, social and community services.
You will play a critical role in supporting participants to build independence, achieve their NDIS goals, and maximise their quality of life.
Key Responsibilities
The NDIS In Home and Social Services Coordinator will:
Coordinate and oversee in-home, social and community services.
Act as the primary point of contact for participants, families, guardians, Support Workers, and allied health professionals.
Support participants to achieve greater independence, capacity building, and community participation goals.
Conduct regular participant visits to ensure supports are delivered safely and effectively.
Complete audits, risk assessments, hazard reports, and incident reports.
Develop, review, and monitor participant support plans, NDIS goals, and risk management strategies.
Coordinate participant intake, onboarding, transitions, and exit planning.
Implement, monitor, and report on Behaviour Support Plans in accordance with NDIS Practice Standards and NDIS Rules.
Prepare and submit reportable incidents to the NDIS Quality and Safeguards Commission.
Complete monthly reporting relating to Regulated Restrictive Practices where applicable.
Monitor participant funding utilisation and collaborate with stakeholders to ensure supports remain financially sustainable.




Ensure participant documentation, service agreements, support plans, and records are accurate, compliant, and regularly reviewed.
Lead and coordinate support teams to deliver high-quality participant outcomes.
Work collaboratively with the Rostering Team to ensure staffing arrangements align with participant needs, SCHADS Award requirements, and NDIS pricing arrangements.
Drive continuous quality improvement initiatives in service delivery.
Complete all administrative and compliance requirements to a high standard.
We Are Looking for Someone with Experience In:
Coordinating in-home, social and community services.
Writing and submitting reportable incidents to the NDIS Commission.
Reporting on the use of regulated restrictive practices.
Interpreting NDIS plans, funding arrangements, and accommodation supports to maximise participant outcomes.
Developing and implementing person-centred support plans.
Understanding the Positive Behaviour Support Framework and Behaviour Support Plans.
Managing complex participant needs within community settings.
Maintaining participant records, service agreements, risk assessments, incident reports, and compliance documentation.
Leading and mentoring support workers within community environments.
Developing strong professional relationships with participants, families, guardians, Support Workers, and external stakeholders.
Applying NDIS Practice Standards, NDIS Rules, Code of Conduct, and safeguarding requirements.
Rostering in accordance with the SCHADS Award.
Identifying opportunities for continuous improvement in service delivery.
Collating participant data and reporting outcomes against funding and service requirements.
Essential Requirements
Minimum 1-2 years' experience in NDIS Services Coordination, or a similar role.
Strong understanding of NDIS funding.
High-level computer literacy and ability to learn new systems quickly.
Excellent written and verbal communication skills.




Working With Children Check.
NDIS Worker Screening Clearance.
Current valid Western Australian Driver's License.
Completion of:
NDIS Worker Orientation Module - Quality, Safety and You
NDIS Supporting Effective Communication Module
New Worker NDIS Induction Module
Desirable Qualifications
Certificate III and/or IV in Community Services, Disability, Aged Care, Mental Health, or related field.
Experience working with complex behaviours in support environments.
