Intereach in Victoria is seeking a Care Partner with case management experience to support participants and their supporters. The role includes accepting referrals and arranging in-home services for CHSP or DVA/HACC-PYP programs.

You will help older people and carers maximise independence and wellbeing at home, upholding wellness, reablement and restorative care. The role is permanent full time and based in Bendigo, Echuca, Maryborough, Swan Hill, Seymour or Gisborne offices.
