About the Role
The successful applicant will be a senior Occupational Therapist within GH at Home at Grampians Health Ballarat.
The role involves providing clinical leadership and supervision, managing a diverse and challenging clinical caseload in a subacute setting.
Responsibilities span the Geriatric Evaluation and Management Unit (GEM) and the subacute wards of Inpatient Rehabilitation and palliative care, supporting seamless transition and discharge home.
Key Responsibilities
Provide advanced assessment, treatment and care planning for complex clients
Supervise and mentor Grade 1 and 2 staff within the GH at Home team
Lead or support service development, quality improvement and evidence‐based practice
Support operational duties such as caseload allocation, data review and rostering
Act as a delegate for the OT Manager as required and lead internal communications to build partnerships across services
Skills & Experience
Registered Occupational Therapist with AHPRA
Minimum seven years of post‐graduate clinical experience, particularly with older people and complex discharge planning
Robust knowledge in managing complex needs in a community setting
Proven experience supervising and mentoring allied health staff
Post‐graduate qualification in a relevant area or currently pursuing one
Experience leading quality improvement or research projects
