With regular mentorship, supervision and a highly supportive team culture, you'll always have the guidance and encouragement to succeed.Key ResponsibilitiesProvide client-centred care to elderly individuals and people living with disabilities in their homesConduct comprehensive assessments using standardised tools and clinical observationDevelop personalised intervention plans and set meaningful, achievable goalsRecommend appropriate assistive equipment and home modifications to maximise client independencePrepare clear and timely reports using our structured templates and clinical toolsApply sound clinical reasoning across a wide range of conditions and presentationsEngage in ongoing professional development and integrate new skills into clinical practiceWork collaboratively within a multidisciplinary team to achieve the best possible client outcomes.Collaborate with multiple stakeholders, including families, support coordinators, and other health professionals to ensure holistic careKey RequirementsCurrent AHPRA registration (or eligible for registration for new graduates)Valid Australian Driver's Licence and access to your own vehicleCurrent Police Check, NDIS Worker Screening Check, and Working with Children Check (or willingness to obtain)Excellent written and verbal communication skillsAbility to work both independently and as part of a team
