Rochester and Elmore District Health Service"To be a trusted leader in our rural health care"Full Time | Ongoing - Rochester & Elmore District Health Service (REDHS)Lifestyle Team LeaderRochester & Elmore District Health Service is seeking an enthusiastic and compassionate lifestyle leader to lead the delivery of engaging lifestyle, wellbeing and social support programmes across Yalukang Aged Care.This is an exciting opportunity for an experienced lifestyle professional who is passionate about enhancing the lives of older people through meaningful activities, community connections and person‐centred care.Working closely with residents, families, volunteers and multidisciplinary teams, you will coordinate innovative lifestyle programmes that support independence, wellbeing, identity and social engagement.
You will also provide leadership and support to a dedicated team of Lifestyle Assistants while contributing to service development and continuous improvement.About the RoleLead, coordinate and evaluate lifestyle and wellbeing programmes for aged care residentsDevelop personalised lifestyle plans that reflect each person's interests, goals, culture and preferencesSupport social, physical, emotional, cognitive, spiritual and community engagement opportunitiesCoordinate special events, outings, volunteer programmes and community partnershipsCollaborate with Allied Health, Nursing, Social Support and Transition Care Programme teamsProvide day‐to‐day leadership, supervision and mentoring to Lifestyle AssistantsEnsure compliance with the Aged Care Act ****,
Aged Care Quality Standards and REDHS policiesDrive continuous improvement initiatives focused on resident wellbeing and quality of lifeAbout YouCertificate III or IV in Leisure & Lifestyle, Diversional Therapy or equivalent qualificationDemonstrated experience coordinating lifestyle, recreational or wellbeing programmes for older peopleStrong understanding of person‐centred care and aged care practiceExperience leading and supporting staff in a team environmentExcellent communication, organisational and relationship‐building skillsUnderstanding of dementia‐friendly and inclusive engagement approachesAbility to connect with residents, families, volunteers and community groupsCommitment to creating meaningful experiences that enhance quality of lifeView the Position Description that contains full details of the position, or go to working-with-redhs/current-vacancies/Rochester and Elmore District Health Service (REDHS) is situated in Rochester which is the gateway to the mighty Murry River region and close to the regional centres of Echuca, Bendigo and Shepparton.We are a Small Rural Health service, providing a broad range of acute, residential aged care,
