Support Coordinator
Motor Neurone Disease South Australia (MNDSA)
Motor Neurone Disease South Australia is seeking an experienced and compassionate Support Coordinator to join our Client Services team.
This unique role combine hands-on client coordination across NDIS and Aged Care programs.
Reporting to the Team Leader you will lead our Support Coordination team while directly supporting people living with Motor Neurone Disease (MND) and their families to access timely, high-quality, person-centred supports.
About the Role
This is a diverse clinical coordination role with responsibilities across NDIS and Commonwealth Home Support Programme (CHSP) and Support at Home under My Aged Care (MAC) services.
Client Coordination
Deliver high-quality, person-centred Support Coordination for individuals living with MND
Assess client needs and develop responsive, future-focused support plans
Coordinate services across NDIS, CHSP, My Aged Care and health systems
Assist clients to access funding and navigate complex service systems
Liaise with health professionals, service providers and external stakeholders
Advocate on behalf of clients with NDIS planners, aged care providers and health services
Provide education and support to clients, families and carers
Manage caseloads
Foster a collaborative, values-driven team culture
Service Development & Collaboration
Work collaboratively with MNDSA Allied Health, Equipment Services, Advice & Coordination and Volunteers
Contribute to NDIS and My Aged Care audit readiness, quality improvement,
projects and organisational initiatives
Support continuous improvement of service delivery across both NDIS and Aged Care programs
About You
You are an allied health qualified with strong capability and a commitment to person-centred, rights-based practice.
Essential
Tertiary qualification in nursing, social work, psychology, OT or other allied health
AHPRA registration (or eligibility, where applicable)
Demonstrated experience in NDIS and or CHSP/My Aged Care)
Experience in assessment, care planning, referral and implementation
Strong team work and communication skills
Excellent organisational skills and ability to manage competing priorities
High level of computer literacy
Current South Australian Driver's Licence
Desirable
Previous coordination experience
Experience working across both NDIS and Aged Care systems
Understanding of progressive neurological conditions and their impact on families and carers
