The Role:
Undertake assessments, create care plans and nursing interventions in partnership with consumers and/or their representatives, in line with consumer needs and preferences, qualified guidelines and organisational processes.
Liaise with medical, allied health, pharmacies,
service providers and equipment providers to ensure a holistic, comprehensive delivery of care.
Document and report care interventions, observations, changes in needs and consumer feedback in a accurate, objective and efficient manner.
Coordinate staff as required on a shift-by-shift basis
Assist in the review and dissemination of clinical policies, procedures and assessment tools under the direction of the Clinical Nurse Manager.
Provide clinical advice and education to persona l care staff to ensure appropriate care.
The successful candidate will have:
Registered Nurse with current AHPRA registration
Experience ideally in an aged care working in a residential setting
Able to work AM, PM and Night shifts - Monday - Sunday
Able to work across Permanent shifts - 10 shifts per fortnight
Sound understanding of the Aged Care Accreditation Standards
Current National Police Clearance and NDIS screening
Current influenza and COVID vaccinations
