An exciting opportunity is available for an Occupational Therapist to join a growing Community Allied Health team supporting older adults and people living with disabilities to maximise independence at home.
You will provide client-centred occupational therapy services across the community, working with clients, families and carers to develop practical solutions that improve safety, independence and quality of life.
This role offers a varied caseload across aged care and disability services, with the opportunity to work collaboratively alongside a multidisciplinary team and Allied Health Assistants.
Caseload:
Frail aged clients and people living with disabilities
Home Care Package (HCP) clients
Commonwealth Home Support Programme (CHSP) clients
HACC PYP clients
NDIS clients (minimal exposure)
Functional assessments
Home safety assessments
Falls prevention and risk management
Assistive technology and equipment prescription
Home modifications and environmental assessments
Wheelchair and scooter assessments
Activities of daily living and independence support
What You'll Be Doing:
Conduct occupational therapy assessments within clients' homes and community settings.
Provide recommendations for home modifications, assistive technology and equipment solutions.
Develop personalised goals and care plans to support clients to remain independent at home.
Complete functional assessments, clinical reports and funding documentation.
Collaborate with clients, families, carers and the wider Allied Health team to deliver holistic care.
Work alongside Allied Health Assistants to implement therapy programs and equipment trials.
Provide supervision and support to Grade 1 Occupational Therapists, students and Allied Health Assistants.

Requirements:
Current AHPRA registration as an Occupational Therapist
Community occupational therapy experience highly regarded
Experience working with older adults and/or people living with disabilities
Strong knowledge of home modifications and Australian Building Standards
Experience prescribing assistive technology and equipment
Experience with wheelchair and scooter assessments desirable
Understanding of funding schemes such as CHSP, HACC PYP, NDIS, Geat2GO and SWEP desirable
Ability to work independently with minimal supervision
Current driver's licence and willingness to complete home visits
