We Care NSW is a provider of specialist Aboriginal disability, mental health and OOHC services.

About the role
We Care NSW encourages applications from Aboriginal and Torres Strait Islander people, people living with adisability and women
We are seeking an individual who will be responsible for delivering quality Support Coordination for National Disability Insurance Scheme (NDIS) participants with high complex needs and/or experiencing high levels of risks.
The outcomes for the role and participants are to reduce complexity, build capacity and resilience, and assist connection with the range of supports required for the participant.
A key challenge of this role involves engaging and sustaining meaningful stakeholder relationships to ensure a high standard of person-centered service provision for participants.
Your success in the role is defined by your ability to successfully navigate complex family dynamics and relationships to ensure participants receive person centered and responsive supports.




You recognise and appreciate the inherent value of all people and use your well-developed conflict resolution skills to build stakeholder relationships that support participants' capacity building.
Your passion is working with people.
Part Time role available 20 hours per week preferably worked across 3 - 4 days
Skills and Experience
Current clear NDIS Worker Screening Check.
Current NSW Drivers Licence.
Certificate IV in Disability Services or similar and/or equivalent knowledge and experience within the sector.
Demonstrated experience in coordinating responsive, individualised support to participants with complex needs and their families.
A sound understanding of NDIS legislation, pricing and procedural guidelines related to the provision of supports for NDIS participants.
Highly developed verbal communication and interpersonal skills.
Demonstrated well developed conflict resolution and negotiating skills
Demonstrated skill in researching information and developing effective and cutting-edge support options.
Demonstrated well developed administrative and written skills.
