Western Health is seeking a Grade 2 Occupational Therapist to join the Western Health at Home team within Rapid Allied Health and CB TCP services in Melbourne.
The role focuses on delivering person-centred home-based care, preventing hospital presentations and supporting secure discharge to maximise independence for older adults.
You will provide advanced clinical expertise, supervise junior staff, and contribute to service improvements within an interdisciplinary team across community settings.
