Occupational Therapist Northern & Western Melbourne

We're looking for an Occupational Therapist to join our supportive Allied Health team in a permanent ongoing role ******** FTE) supporting clients across Melbourne's northern and western metropolitan regions. This community-based role offers flexibility, autonomy and the opportunity to make a meaningful difference in the lives of older Australians.
Availability to work Fridays is essential.
About the role
This is primarily a community-based position where you'll support older Australians to maintain their independence, safety and quality of life at home.
You will provide:
Home and environmental assessments
Falls prevention and risk management
Equipment prescription and mobility assessments
Home modification recommendations
Cognitive and functional assessments
Rehabilitation in the home
Client-centred care planning and goal setting
Collaboration with other health professionals to deliver holistic care
The majority of your work will be community-based, with a small residential aged care component.

About you
You'll bring:
Bachelor of Occupational Therapy (or equivalent)
Current AHPRA registration, or eligibility to obtain registration
Current driver's licence and reliable vehicle
Strong communication and relationship-building skills
Ability to work independently while remaining connected to a collaborative team
Passion for delivering person-centred care
