About the Role
As the Case Manager – Families & Single Adults Program, you will provide trauma-informed, person-centred case management to families and single adults aged 25 years and over who are experiencing or at risk of homelessness and/or family violence.
Working within Junction Support Services' Specialist Homelessness Service, you will support clients to achieve safe, stable housing through comprehensive assessments, tailored support plans, advocacy and coordinated referrals, empowering individuals and families to build safer and more independent futures.
This role is a part-time ***** hours per fortnight fixed-term contact until 30 June ****, located in our Wangaratta location.
What You Can Expect
Reporting to the Homelessness Team Manager, you will:
Conduct comprehensive needs and risk assessments for families, single adults and accompanying children.
Develop, implement and regularly review individualised support plans that promote housing stability and recovery.
Support families to strengthen life skills, parenting capacity and independence.
Work collaboratively with clients to identify goals and empower them to achieve sustainable outcomes.
Coordinate referrals to housing providers, health services, education and training, legal services, family violence services and other community supports.
Advocate for clients to access safe and appropriate housing options.




Work closely with the Homeless Children's Specialist Support Service and other key stakeholders to ensure children's needs are identified and addressed.
Maintain accurate client records, case notes, support plans and reports in accordance with organisational and funding requirements.
Participate in professional supervision, ongoing learning and development, and contribute positively to a collaborative team environment.
Participate in the organisation's after-hours on-call roster as required.
About You
You'll bring:
A tertiary qualification in Social Work, Welfare, Behavioural Science or a related discipline, or significant demonstrated experience in a similar role.
Experience working with people experiencing homelessness, family violence or other complex social issues.
Sound knowledge of trauma-informed, strengths-based and person-centred practice.
The ability to undertake comprehensive assessments and develop effective case management and support plans.
An understanding of child development and the ability to support families in strengthening parenting skills.
Knowledge of the impacts of family violence, homelessness and complex trauma on individuals and families.
Strong communication, advocacy, report writing and relationship-building skills.




The ability to work collaboratively with a broad range of internal and external stakeholders.
Excellent organisational skills with the ability to manage competing priorities.
