We are seeking a motivated
Senior Dietitian to join our multidisciplinary
team.

Fremantle RITH 0.7 FTE.
Day in the life of a Rehabilitation in the Home (RITH)
therapist
As part of the RITH team, you will play a key role in delivering
subacute rehabilitation to patients in the comfort of their own
homes.
No two days are the same.
You'll receive referrals
from allied health ward staff and cross referrals from team
members, with allocations made daily to your caseload.
Working in
an interdisciplinary model, you'll complete comprehensive
multidisciplinary assessments before working with the patient to
establish their rehabilitation SMART goals.
In this dynamic role, you will work with a diverse range of
patients across gerontology, complex neurology, general medical and
surgical, and orthopaedic specialties.
As part of a
multidisciplinary team, you will contribute to case coordination
and discharge planning, facilitate access to appropriate community
supports, and provide essential carer education, coaching, and
safety planning.
Through collaborative, person-centred practice,
you will help ensure patients receive safe, coordinated care that
supports their independence and enables them to remain living at
home.
You'll have access to a work vehicle, laptop, mobile phone and
safety systems to ensure your home visits run smoothly and
safely.
South Metropolitan Health Service (SMHS) RITH is a
well-established early discharge service.
We currently have 8
bases across the metropolitan region.
You will be supported
by a Coordinator and your disciplinary and interdisciplinary peers,
for the more challenging clinical and non-clinical aspects of your
role.
Regular professional development is part of a standard
week, with opportunities for quality improvement, innovation and

The within-team support
from Physiotherapists, Occupational Therapists, Social Worker,
Speech Therapist, Medical Registrar and Allied Health Assistants
will keep you inspired and keen to go to work each day.
Who We are Looking For
We welcome applications from Dietitians who demonstrate:
Robust clinical reasoning, assessment and problem-solving
skills, with extensive experience in both acute settings and
rehabilitation.
Community experience is a bonus.
A genuine interest in delivering hospital-level rehabilitation
in a community setting.
Confidence working both autonomously and collaboratively within
a multidisciplinary team.




Excellent communication skills and the ability to thrive in
dynamic, mobile and virtual care environments.
A commitment to safe, person-centred and evidence-based
practice.
Passion for home-based, patient-centred care and holistic
support
