Key Responsibilities

Client Care Management

- Monitor service quality and client outcomes.

Staff Coordination

- Schedule and coordinate care workers.

- Match clients with appropriate support staff.

- Monitor staff performance and service delivery.

- Provide guidance and support to care teams.

Communication & Documentation

- Maintain accurate client records.

- Liaise with families, healthcare professionals, and service providers.





- Prepare reports and documentation as required.

- Ensure compliance with organizational policies.

Compliance & Quality Assurance

- Monitor adherence to care standards and regulations.

- Participate in audits and quality improvement initiatives.

- Investigate incidents and implement corrective actions.

Qualifications

Essential

- Diploma or Bachelor's degree in Nursing, Community Services, Social Work, or related field.

- Minimum 2 years experience in aged care, disability, or community services.

- Strong organizational and communication skills.
