As a Care Partner, you'll play an important role in helping older people live with dignity, independence, and choice. Working alongside clients, families, and service providers, you'll develop tailored care plans that reflect each person's goals, preferences, and circumstances.

These aren't set-and-forget care plans. You'll regularly review and adapt supports as clients' needs change, ensuring their care remains aligned with both aged care standards and what matters most to them.

You'll support clients who may have experienced homelessness, mental health challenges, trauma, or other complex life circumstances. Using a person-centred and trauma-informed approach, you'll build trusted relationships, advocate for clients, and help them navigate their aged care journey with confidence and dignity.

This role would also suit someone currently working at a Coordinator level ready to step up.

A day in the role could see you:

- Coordinating care for clients with moderate, high, and complex support needs, ensuring they receive the right services at the right time.
- Conducting comprehensive assessments to understand clients' goals, strengths, circumstances, and preferences.
- Developing, reviewing, and updating person-centred care plans in line with client needs and regulatory requirements.
- Coordinating service delivery under the Support at Home framework while managing individual budgets.
- Building strong relationships with clients, families, providers, and internal stakeholders.
- Maintaining accurate and confidential records in line with privacy, compliance, and documentation standards.

You'll stand out if you have:

- A Certificate IV in Aged Care, or you're an Enrolled Nurse.
- Experience in a Coordinator, Case Management, Care Partner, or similar role (highly regarded but not essential).
- Experience within aged care, community services, mental health, or supporting vulnerable populations.




- The ability to build meaningful relationships and support clients using a person-centred, trauma-informed approach.
- Strong communication, problem-solving, and stakeholder engagement skills.
- Excellent computer literacy and strong attention to detail when maintaining client records and documentation.
