The Role: Clinical Expertise Meets Leadership As a Clinical Care Coordinator, you will be the clinical heart of our expanding service, ensuring high-quality, compliant care across our growing community.

- Lead Care Planning:



Conduct home assessments and design individualised care plans that empower independence.

- Mentor & Supervise: Provide specialist nursing advice and leadership to our dedicated, growing team of care staff.

- Hands-On Clinical Care: Manage wound care, medication administration, and specialised treatments.

- Coordinate Continuity: Act as the vital link between clients, families, hospitals, and allied health professionals.

- Advocate: Support NDIS and My Aged Care compliance to ensure every client hits their personal goals.

Role Requirements We are looking for professionals who are excited by growth, innovation, and clinical excellence.

- Credentials: Current AHPRA Registered/Enrolled Nurse Registration is essential.

- Clinical Savvy: Strong foundational nursing skills with a "quality-first" mindset.

- Communication: Exceptional verbal and written skills—you’ll be building relationships every day.

- Tech Literacy: Proficient in Microsoft Office and Excel.

- Mobility: A valid Australian Driver’s License and access to your own reliable vehicle.
