Lead clinical excellence and make a meaningful difference as a Clinical Care Coordinator / Nurse Mentor with a respected aged care provider in Adelaide's southern suburbs.
This is an exciting leadership chance for an experienced Registered Nurse who is passionate about delivering exceptional resident-centred care while mentoring and supporting a dedicated clinical team. Working Monday to Friday, you'll play a key role in ensuring outstanding clinical outcomes, driving best practice, and fostering a positive and collaborative care environment within a well-established 125-bed residential aged care facility.
- Lead and support a high-performing clinical team to deliver exceptional, resident-centred care.
- Mentor, coach and develop nursing staff while promoting clinical excellence and continuous improvement.
- Build strong relationships with residents, families and multidisciplinary stakeholders to ensure the highest standards of care.
YOU | You are an experienced Registered Nurse with a passion for aged care leadership and mentoring others to deliver exceptional clinical outcomes. You thrive in a hands-on environment, communicate with empathy and professionalism, and are committed to maintaining best practice and compliance across all aspects of clinical care. You will also bring:
- Current unrestricted Registered Nurse Division 1 registration with AHPRA.
- Minimum one year's experience as a Care Coordinator, Clinical Care Coordinator, Nurse Mentor or similar leadership role within residential aged care.
- Proven ability to mentor and support Registered Nurses, Enrolled Nurses and Personal Care Workers to build capability and strengthen team performance.
- Strong knowledge of aged care documentation, care planning, accreditation standards and clinical governance.
- Excellent communication, leadership and stakeholder management skills with a calm and solutions-focused approach.
- Current First Aid and CPR certification.
- A valid NDIS Worker Screening Clearance.
- A current National Police Clearance (or ability to obtain one).
- Full Australian working rights.
- Postgraduate qualifications in aged care (desirable).
THE ROLE | Reporting to the Clinical Care Manager and supported by the General Manager, you will oversee the day-to-day clinical operations of this well-established residential aged care facility. You'll provide leadership and guidance to the clinical team while ensuring residents receive safe, person-centred care that meets contemporary best practice standards. Key responsibilities include:
- Leading the delivery of high-quality clinical care and ensuring compliance with aged care standards.
- Overseeing the development, implementation and evaluation of individualised care plans.
- Coordinating multidisciplinary care with allied health professionals, GPs, hospitals and families.
- Managing complex clinical situations, specialist referrals and escalation of resident health concerns.
- Supporting staff development through onboarding, coaching, training and performance management.
- Ensuring clinical documentation, assessments and care plans remain accurate, compliant and up to date.
