About the Role As a Clinical Case Manager , you will be responsible for managing a diverse portfolio of clients, ensuring they receive coordinated, high-quality services that align with their individual goals, clinical needs, and funding arrangements. Working closely with clients, families, support networks, and healthcare professionals, you will undertake comprehensive assessments, develop tailored care plans, and continuously monitor outcomes to ensure services remain safe, effective, and responsive.

Key Responsibilities
- Manage an allocated client portfolio, ensuring services are planned,
delivered and reviewed in line with client goals and funding requirements
- Conduct structured assessments and develop tailored care plans reflecting individual needs, risks and preferences
- Coordinate end-to-end client onboarding, ensuring timely intake and smooth transition into service delivery
- Monitor client funding utilisation, identifying underspend or overspend and adjusting to maintain financial sustainability
- Build effective relationships with clients, families, funding bodies and support coordinators
- Manage incidents and complaints in accordance with organisational frameworks
- Maintain accurate, audit-ready documentation and identify opportunities to improve service delivery
About You You are a compassionate and organised skilled with strong clinical knowledge and a passion for delivering person-centred care. You thrive in a fast-paced environment, can confidently manage competing priorities, and have excellent communication and stakeholder engagement skills. You are committed to supporting clients to achieve the best possible outcomes while ensuring services remain compliant, sustainable, and clinically appropriate.
- Tertiary qualification in health, community services, nursing or a related discipline (or equivalent experience)
- Current National Police Clearance (or willingness to obtain)
- Working With Children Check, where applicable to the role
- NDIS Worker Screening Clearance (or willingness to obtain)
- A valid driver's licence
- Evidence of current mandatory immunisations
- Full right to work in Australia
