Join Hunter New England Local Health District's innovative Virtual Heart Failure Support Program (VHFS) and help shape the future of chronic disease management. Working within a district-wide virtual service, you'll provide specialist support to people living with heart failure as they transition from hospital to home, improving outcomes while helping reduce avoidable hospital admissions.

Where you'll be working The Virtual Heart Failure Support Program is a team-oriented district-wide team comprising experienced Heart Failure Clinical Nurse Specialists and a Transitional Nurse Practitioner. Although team members are based across multiple Hunter New England locations including Newcastle, Maitland, Manning and Tamworth, the service operates as one integrated virtual team.

Through regular virtual huddles, multidisciplinary collaboration and shared models of care, you'll support patients across metropolitan, regional and rural communities, ensuring equitable access to specialist heart failure services regardless of where they live.

About the role

As the Clinical Nurse Specialist, you'll deliver virtual post-discharge care to people living with heart failure, supporting them through recovery and empowering them to manage their condition confidently at home.

Your role will include

- Delivering telephone and telehealth consultations.

- Providing patient education and self-management support.

- Coordinating care across hospital, community and primary care services.

- Supporting medication optimisation and evidence-based heart failure management.

- Working closely with GPs, cardiologists, cardiac rehabilitation and multidisciplinary teams.

- Contributing to service improvement, education and quality initiatives across the district.

About you

You'll be an experienced Registered Nurse with a passion for cardiology and delivering person-centred care. You'll thrive working independently while remaining connected within a collaborative virtual team.

You'll bring

- Experience in acute or community cardiology or working with patients with chronic disease.

- Flexible and adaptable with the ability to work autonomously, deliver patient-centred care, communicate effectively with a broad range of stakeholder





- Ability to confidently utilise digital technologies to provide high-quality virtual care.

- Excellent communication and relationship-building skills.

- Confidence using digital technology to deliver virtual care.

- Strong clinical judgement, organisation and the ability to work autonomously.

- A genuine commitment to improving patient outcomes through innovative models of care.

Essential Requirements

- Current registration as a Registered Nurse with AHPRA.

- Eligibility to drive in NSW
