The Role As Community Care Manager, you will lead an established Support at Home service, supporting your team to deliver high-quality, person-centred care to community clients.

Key responsibilities include

- Leading and supporting Care Partners and care staff
- Overseeing care plans, budgets and service reviews




- Supporting complex clients and escalations
- Monitoring compliance, quality, documentation and risk
- Managing incidents, complaints and performance matters
- Driving continuous improvement and staff development
- Working closely with clients, families and broader leadership teams

About You You may already be a Care Manager, or an experienced Senior Care Partner / Team Leader ready to step into a broader management role.

You will ideally have

- Strong Home Care / Support at Home experience
- Experience leading or mentoring community care teams
- Strong knowledge of Support at Home and the Aged Care Quality Standards
- Experience with care plans, budgets and client reviews
- Strong understanding of quality, compliance and risk
- Relevant Community Services, Aged Care or Nursing qualification
