Join Our Dedicated Home Care Team as a Registered Nurse!

Are you a compassionate and experienced Registered Nurse looking to make a meaningful difference in the community?

We are seeking a reliable Registered Nurse to deliver high-quality, person-centred nursing care to clients in their own homes. This is a casual position offering approximately 3–4 days of work per week.

You will work independently in the community while receiving ongoing support from our care coordination and management teams.

Position Summary:

- Provide safe, high-quality and person-centred nursing care in clients’ homes.
- Conduct comprehensive nursing, clinical and risk assessments.
- Develop, implement and review individualised care plans.
- Provide wound care and wound management.
- Provide catheter, continence, stoma and bowel care within your scope of practice.
- Administer medications, injections and prescribed treatments.
- Monitor clients with chronic and complex health conditions.
- Identify and respond appropriately to changes in clients’ health.
- Educate and support clients, families and carers.
- Maintain accurate and timely clinical documentation and progress notes.
- Liaise with GPs, hospitals, allied health professionals and other service providers.
- Follow infection control, incident reporting and workplace health and safety procedures.
- Travel between clients as required
- Perform other nursing duties within the Registered Nurse’s scope of practice.

Ideal Candidate:

- Holds a recognised Bachelor of Nursing degree or equivalent qualification.
- Holds current unrestricted AHPRA registration as a Registered Nurse.
- Has previous experience in community nursing, disability care, aged care or home care.
- Demonstrates experience in clinical assessments, care planning, wound care, catheter care and medication management.




- Can work confidently and independently in a community-based workplace.
- Has excellent communication, clinical decision-making and documentation skills.
- Is compassionate, dependable, organised and respectful of clients from diverse backgrounds.
- Demonstrates cultural awareness and the ability to provide respectful, person-centred care to clients from diverse cultural and linguistic backgrounds.
- Must possess a valid Australian driver’s licence and have access to a reliable, insured vehicle.
- Must be willing to travel throughout Melbourne’s Western and Northern suburbs.
- Holds a current National Police Check.
- Holds current First Aid and CPR certificates.
- Holds an NDIS Worker Screening Check or is willing to obtain one.
- Has valid Australian working rights.
- Is available to work approximately 3–4 days per week.
