At Healthy Communities Foundation Australia, we’re transforming the way healthcare is delivered to Residential Aged Care Facilities through an innovative hybrid model of care.
Our GPs provide predominantly virtual consultations, supported by an experienced multidisciplinary team, with quarterly visits to each allocated facility to maintain strong clinical relationships and continuity of care.

What the role offers
- Work remotely from anywhere in Australia, with clinics available Monday to Friday, 9:00am – 5:00pm.

The Role
As part of our Virtual Aged Care Service, you will:
- Provide comprehensive GP services to residents of Residential Aged Care Facilities.
- Conduct virtual consultations supported by onsite nursing staff.
- Visit your allocated facilities once per quarter.
- Collaborate with an experienced multidisciplinary team to deliver high-quality, patient-centred care.
- Build ongoing relationships with residents, families and facility staff
What we’re looking for
We are seeking compassionate and skilled GPs with a passion for rural and remote health. To be successful in this role, you will possess:
- An ABN
- A Fellowship of the Royal Australian College of General Practitioners (RACGP).
- Current, unconditional medical registration with AHPRA.
- A strong understanding of the unique challenges of rural and remote health settings.
- Proven expertise in diagnosing and managing a broad range of general medical conditions.
- Experience in aged or palliative care.
- A patient-focused approach with genuine empathy and a commitment to providing respectful and culturally sensitive care.
- Exceptional communication skills, with the ability to convey complex medical information clearly and empathetically.
- Proficiency in using virtual communication platforms and electronic health records.
- The ability to work both independently and as part of a remote team.
