Key Responsibiliti**

- esProvide personal care, domestic assistance and social support to clients in their home
- s.Support clients with daily living activities, including meal preparation, shopping, cleaning and attending appointment
- s.Build positive and respectful relationships with clients and support their individual preferences and need
- s.Encourage clients to remain socially connected and participate in recreational and community activitie
- s.Monitor client wellbeing and promptly report any changes or concern
- s.Maintain accurate client records and documentatio
- n.Work collaboratively with Care Partners and the broader care team to provide high-quality, person-centred suppor
- t.Deliver services in accordance with the Aged Care Quality Standards and organisational policie

Skills & Qualificati**
- onsCertificate III in Individual Support (Ageing) or equivalent qualification (Essentia
- l).Minimum 1 year's experience supporting older people or vulnerable members of the communi
- ty.A warm, compassionate and respectful approach, with a genuine interest in supporting othe
- rs.Strong communication and interpersonal skills, with the ability to build trusting relationships with clients and their famili
- es.Understanding of person-centred care and the individual needs of older peop
- le.Ability to work independently while also contributing positively as part of a multidisciplinary te
- am.Current First Aid and CPR Certificate or willingness to obta
- in.Current National Police Check (or willingness to obtai
- n).Current NSW Driver Licence and access to a reliable vehicle (require
- d).Additional language skills are highly valued, particularly Chinese, Greek or Arabic, enabling you to connect with and support clients from diverse cultural backgroun
