Role Overview
We are seeking a compassionate and experienced Intake and Brief Intervention - Senior Case Worker to join our Social and Emotional Wellbeing (SEWB) team in Mildura.
This role provides the first point of contact for Aboriginal and Torres Strait Islander community members accessing MDAS Social and Emotional Wellbeing services. Working within a culturally safe, trauma-informed and strengths-based framework, the role focuses on conducting comprehensive intake assessments, mental health and psychosocial screening, risk assessment,
brief interventions and short-term case management for individuals experiencing mental health, social and emotional wellbeing concerns.
The Intake and Brief Intervention Practitioner supports clients presenting with acute psychosocial distress, mental health concerns and complex wellbeing needs by providing timely assessment, care coordination, safety planning and referral pathways. Working collaboratively within a multidisciplinary team, the role contributes to early intervention, improved access to services and positive client outcomes through coordinated, wraparound care.
Key Responsibilities
- Conduct comprehensive intake, psychosocial, mental health and risk assessments, including crisis assessment, safety planning and MARAM screening where required.
- Provide culturally safe, trauma-informed brief interventions and short-term case management to support clients experiencing mental health and social and emotional wellbeing challenges.
- Develop, implement and review individual treatment plans, ensuring client goals, strengths and support needs are identified and regularly monitored.
- Coordinate referrals, advocacy and care pathways with internal programs, health services and external agencies to ensure a holistic and integrated approach to care.
- Respond appropriately to high-risk presentations and escalate concerns in line with clinical governance, risk management and organisational procedures.
- Maintain accurate client records,
data collection and reporting requirements while contributing to continuous quality improvement and multidisciplinary team activities.
Key Selection Criteria
- Demonstrated experience conducting mental health, psychosocial and risk assessments, including intake, screening and triage processes.
- Proven ability to provide effective case management, brief interventions and coordinated care for individuals experiencing mental health and wellbeing challenges.
- Sound knowledge and application of trauma-informed, recovery-oriented and culturally safe practice frameworks.
- Experience working with Aboriginal and Torres Strait Islander peoples and communities, with a commitment to delivering culturally responsive services.
- Strong communication, interpersonal and stakeholder engagement skills, with the ability to build collaborative relationships across services and sectors.
- Ability to work independently and within a multidisciplinary team while managing competing priorities, maintaining professional standards and achieving service outcomes.
Qualifications
- Tertiary qualification in Psychology, Social Work, Nursing, Mental Health, Community Services or a related discipline.
- Minimum two (2) years' experience in mental health, social and emotional wellbeing, counselling, case management or a related field.
- Relevant registration with the appropriate qualified body, where applicable.
Other Requirements
- Current Employee Working with Children Check.
- Current Driver's Licence.
