Role Description: We are seeking an experienced and highly capable Level 3 Specialist Support Coordinator to join our growing organisation on a contract basis.
The position is based in Braybrook, VIC, with flexible and hybrid working arrangements available, including the opportunity to work from home following successful completion of the probation period.
We are looking for someone with strong NDIS industry experience, excellent case management skills and relevant qualifications who can support participants with complex and high-risk needs.
Key Responsibilities:
- Deliver Level 3 Specialist Support Coordination to NDIS participants with complex and high-risk needs.
- Assist participants to understand, implement and maximise their NDIS plans.
- Conduct needs assessments and develop, implement and review appropriate support strategies.
- Connect participants with suitable providers, allied health professionals, community services and mainstream supports.
- Coordinate supports across multiple providers and stakeholders.
- Identify and manage complex risks, safeguarding concerns,
service gaps and barriers.
- Monitor participant progress against their NDIS goals and available funding.
- Advocate for participants and support greater choice and control.
- Provide guidance regarding NDIS processes, funding and service agreements.
- Maintain accurate case notes, progress reports and compliance documentation.
- Build strong professional relationships with participants, families, guardians, nominees and service providers.
- Resolve participant and service-related issues.
- Ensure services are delivered in line with NDIS requirements, organisational policies and quality and safeguarding standards.
Experience and Qualifications: Applicants must have demonstrated experience in NDIS Support Coordination, preferably including Level 3 Specialist Support Coordination and complex participant cases.
Applicants should have completed, or be currently completing, one of the following:
- Bachelor of Social Work
- Bachelor of Psychology
- Bachelor of Occupational Therapy
- Bachelor of Nursing
Other relevant qualifications may be considered where supported by significant disability, community services or NDIS experience. The successful applicant should also demonstrate:
- Strong knowledge of the NDIS framework and participant funding.
- Experience in case management, assessment, planning and support coordination.
- Strong understanding of NDIS reporting requirements and safeguarding obligations.
- Excellent interpersonal and relationship-building skills.
- Strong written and verbal communication skills.
- Ability to explain NDIS processes clearly to participants and families.
- Robust stakeholder management skills.
- Good digital literacy and ability to use case management systems and NDIS-related portals.
- Ability to work autonomously and manage competing priorities.
- Strong documentation and organisational skills.
Requirements:
- Demonstrated NDIS Support Coordination experience.
- Level 3 Specialist Support Coordination experience highly regarded.
- Experience supporting complex or high-risk participants.
- Strong written and verbal communication skills.
- Current NDIS Worker Screening Check.
- Current National Police Check.
- Working with Children Check, where applicable.
- Valid driver’s licence.
- Access to a reliable vehicle.
- ABN for contract-based engagement.
- Appropriate professional insurance for contract work.
