Warm Embrace Support Services is growing, and we're looking for a passionate and experienced Support Coordinator to grow with us.

We're not simply looking for someone to fill a position.

We're looking for someone who genuinely cares about people, takes the time to understand each participant as an individual, and believes Support Coordination should be about advocacy, connection, trust, and achieving meaningful outcomes.

Our Support Coordination department is still growing, making this a unique opportunity. You won't simply inherit an established caseload you'll have the opportunity to help shape and grow the department alongside us.

For the right person, there is genuine potential to grow your caseload, transition into casual or permanent employment, and potentially step into a leadership role within our Support Coordination team as we continue to expand.

About the Role

As a Support Coordinator, you will:

Support participants to understand and make the most of their NDIS plans.

Build genuine relationships with participants and their families.

Connect participants with services and supports that genuinely suit their individual needs.

Advocate strongly for participants when barriers arise.

Coordinate with allied health professionals, plan managers, providers and other stakeholders.

Monitor participant budgets and support utilisation.

Maintain accurate case notes, documentation and records.

Help participants build confidence, capacity, choice and control.

Work collaboratively with our growing Warm Embrace team.

Who We're Looking For

We're looking for someone who is:





- Experienced in NDIS Support Coordination (Level 1 and/or Level 2)
- Person-centred, compassionate and genuinely invested in participant outcomes.
- Confident navigating NDIS plans, budgets and services.
- Proactive and willing to pick up the phone, follow things up and advocate when needed.
- Organised and able to manage their own workload.
- A strong communicator who can build meaningful relationships with participants, families and providers.
- Comfortable working independently while remaining part of a close and supportive team.
- Excited by the opportunity to help build something meaningful rather than simply stepping into an existing role.
- Most importantly, we're looking for the right person—not just the right résumé.

Essential Requirements

- NDIS Worker Screening Check
- Blue Card (or willingness to obtain)
- Current First Aid & CPR
- Current Australian Driver Licence and reliable vehicle
- National Police Check
- Robust understanding of the NDIS and Support Coordination
- Previous Support Coordination experience highly regarded
