About The Role

The Child Wellbeing Intake and Assessment Practitioner provides intake, assessment, triage and referral support to children, young people and families experiencing vulnerability, safety concerns and complex needs.

Working within a multidisciplinary environment, you will undertake child-focused assessments, identify risks and protective factors, provide timely responses to presenting concerns, and connect families with the services and supports they need. You will work collaboratively with practitioners from across the community services sector to deliver integrated, coordinated and culturally safe responses that promote safety, wellbeing and positive outcomes for children and families.

This role is ideal for someone who is passionate about early intervention, child wellbeing and family support, and who enjoys working within a collaborative practice environment that supports continuous learning and professional growth.

This role reports to the Team Leader and is offered as a 12-month fixed-term contract within the Bayside Peninsula region. Internal secondment applications are welcome.

Key Responsibilities

- Undertake intake, screening, triage, risk assessment and service planning activities for children, young people and families
- Identify and respond to child wellbeing concerns, vulnerability and family complexities using evidence-informed practice frameworks
- Deliver timely crisis responses, targeted interventions and referrals to appropriate support services
- Work collaboratively with internal and external practitioners to develop coordinated and integrated support plans
- Participate in case consultations,



case conferencing and multidisciplinary practice discussions
- Apply child-centred, trauma-informed and culturally secure approaches to all client interactions
- Support families to navigate the service system and access services that strengthen safety, wellbeing and resilience
- Maintain accurate and timely client records, case notes and reporting requirements
- Contribute to continuous improvement activities, reflective practice and service development initiatives
- Participate in ongoing supervision, training and professional development opportunities

About You

- Tertiary qualification in Social Work, Psychology or a related discipline in accordance with Family Violence Reform Recommendation 209 requirements
- Demonstrated experience in child wellbeing and/or family violence is essential
- Sound knowledge of child development, child wellbeing frameworks and family-centred practice approaches
- Experience undertaking risk assessments and working with children, young people and families experiencing complex needs
- Understanding of the Best Interests Principles Framework and the ability to undertake child-focused assessment and planning
- Knowledge of cultural safety principles and experience working with diverse communities
- Experience within family violence services, integrated family services, child protection, community services or related settings
- Strong written, verbal and interpersonal communication skills
- Ability to work effectively within multidisciplinary and collaborative service environments
- Commitment to reflective practice, continuous improvement and delivering high-quality client outcomes
