East Wimmera Health Service is seeking a Healthy @ Home Care Coordinator on a fixed-term basis to support vulnerable individuals, especially those with complex health needs, to stay healthy at home and avoid unnecessary hospital visits.
Working within a multi‑disciplinary team, you will collaborate with GPs and other services, promote self-management, and help re-engage clients with the health system while ensuring appropriate interventions and clinical support where needed.
