Hunter Recent England Health is recruiting a Nurse Practitioner to join the Residential Aged Care In‑Reach (RAC-I) team in the Hunter region. You will deliver resident-centred acute care within aged care facilities, supporting early intervention and reducing hospital presentations.

The role involves collaboration with geriatricians, medical officers, nurses and allied health staff to provide assessment, diagnosis, treatment and care coordination within an advanced scope of practice.
