Job Summary:

Are you passionate about care? At Continuity Care, we're building something that doesn't yet exist in Australia: a truly coordinated care ecosystem where disability support, aged care and primary health work together around the person, not around the system.

As our Business Development Manager, you'll lead the growth engine behind this vision. You'll grow our client base across NDIS and community aged care, while facilitating the partnerships, referral networks and government relationships that turn a fragmented sector into a connected one. This is a rare prospect to shape an integrated, equity-based care model from the ground up one that identifies risk early, keeps people well at home, and ensures no one falls through the cracks between services.

We're looking for someone who sees care not as a transaction but as a relationship someone who believes equitable, coordinated, person-centred care shouldn't depend on how well a person can navigate a complex system. About Us:

Continuity Care delivers individualised, self-directed disability (NDIS) and community aged care services across six service lines, backed by a mature digital health platform. We're disrupting the traditional health and social care model in Australia connecting primary health, aged care and disability support around the person, catching risk early, and keeping people well at home.

About the Role

This is a build role, not a maintain role. We're looking for a Business Development Manager who's energised by standing up new capabilities from scratch growth engines, stakeholder relationships, tools and playbooks that don't yet exist rather than optimising ones that already do.

You'll own four priorities:

- Client growth – grow our client base across community aged care and NDIS





- Awareness & stakeholder engagement – build community awareness and establish a working relationship with Queensland Government on our integrated care approach

- Platform growth – upsell our online platform to clients and partners, and recruit Associate Providers

- Integrated care & revenue – lead rollout of our Integrated Model of Care and optimise revenue across all streams

Reporting to the Managing Director and Operations Manager, you'll lead the Business Development and Engagement team and be the public face of Continuity Care across expos, forums and government relationships.

What You'll Do

- Build and own a referral pipeline across NDIS supports and aged care (Home Care Packages / Support at Home), with quarterly acquisition targets

- Establish high-yield relationships with Support Coordinators, LACs, Plan Managers, allied health professionals, ACAT/RAS assessors and hospital discharge teams

- Design and run multi-channel demand programs relationship campaigns, email nurture, webinars, content and digital

- Represent Continuity Care at disability expos, aged care forums and sector networks; own our event and community awareness calendar

- Build structured engagement with Queensland Government departments, Primary Health Networks and peak bodies to position Continuity Care as a partner for integrated care

- Develop the platform's value proposition and pricing, and recruit Associate Providers (allied health, nursing, support work) to join it

- Lead market-facing implementation of the Integrated Model of Care, establishing pathways with hospitals, GPs and primary health partners

- Stand up the growth systems that don't exist yet CRM, marketing automation, lead scoring and routing and introduce AI-assisted workflows to boost team velocity

- Build and mentor a high-performing Business Development & Engagement team

- Report weekly pipeline and monthly growth results to the executive





About You

Essential:

- 5+ years' progressive business development, growth or relationship management experience, with significant time in NDIS, aged care or the broader health/community services sector

- A track record of building growth programs and infrastructure not just inheriting mature systems

- Demonstrated achievement of sales or client-growth targets, with strong pipeline management and CRM discipline

- Experience engaging multi-stakeholder buying and commissioning groups with long decision cycles (12+ months), including government

- Solid working knowledge of NDIS funding streams (Core, Capacity Building, Capital) and the Home Care Package / Support at Home framework

- Exceptional communication skills equally comfortable in B2B negotiation, government engagement and compassionate conversations with vulnerable people and their families

- Proficiency with CRM, marketing automation and analytics tools, plus practical experience using AI tools to boost productivity

- Experience building and leading a team, with strong commercial and financial literacy (forecasting, pricing, ROI) and budget/vendor management experience

- A self-starter who can manage a broad territory autonomously, with resilience and sound judgement

Desirable:

- An existing, active network of Support Coordinators, Plan Managers and aged care intermediaries

- Experience selling or onboarding partners to a digital/SaaS platform, network or marketplace

- Experience with account-based marketing, demand generation or multi-channel B2B campaigns

- Experience with government tenders, commissioning processes or health/care sector reform

- A tertiary qualification in business, marketing, health administration or a related field

You'll also need:

- NDIS Worker Screening Check, Working with Children Check (WWCC) and National Police Check (current or willing to obtain)

- A valid Australian driver's licence and access to a reliable, comprehensively insured vehicle for regional travel

- Full working rights in Australia

- A Bachelor's degree in health or social care (or related field)

- Willingness to work some out-of-hours events (time-in-lieu applies)
